How to Submit an External CE Request for Fairview Rehab ServicesComplete the Rehab Services CE Request Form found in the IT HUB1.Login to IT HUB using your Fairview ID and Password. You can access IT HUB by clicking on tab located on the Intranet or click on this link IT hub 2. Click on Order Something /Order for myself 3.Click on “order for myself” 4.Type in search bar: Rehab Services CE Request -Once you have submitted a request the first time, the second time you log in you should see it appear on “My recent items and popular items” 5. Some points to keep in mind while entering the data: a. Review Fairview's travel policy and reimbursement process. b. ***All Fairview employees are required to review Fairview's Travel Policy before requesting travel arrangements to ensure all business related expenses are appropriate, reasonable, and properly documented. You are also required to review the CE guidelines. c. Check the box to confirm you have understood the Fairview Travel policy and CE guidelines. d. The "Training/Travel requested for" field will auto-populate with your name and cannot be altered. Each employee must submit their own request, which will require subsequent approvals before any training/travel arrangement in initiated. e. Indicate (using the drop down menu) if you work in Fairview Rehab Services, Institute of Athletic Medicine or Orthotics and Prosthetics. f. Continue to complete the starred/required fields. 6. When you are finished completing all of the required fields, you can add to cart. 7. Go to your Cart found on top right hand corner; left of your name: Click on cart. 8. Click on View cart or you can proceed to checkout -You will receive an email that your request has been accepted and is being reviewed by you Supervisor/Director. ****This is not an approval from director -After it has been approved by your director you will receive another email. The Continuing Education Department will receive approval and sign you up for the course and arrange for travel if needed. You will receive email confirmation from the CE Coordinator when you have been registered for the course and/or travel. 9. It may take a few seconds to transfer to the confirmation screen after you click the order now button. You will need to type in your site (ie: Ridges, Woodwinds, East Bank) in the "Delivery/Service Location (please include floor/office/room information)." If you want to review your request before checking out you may click on the "edit" button that will return you to the form. 10. You will receive an email that you request has been accepted and is being reviewed by your supervisor/director. After it has been approved you will receive another email. The CE department will sign you up for the course and arrange for travel if needed. You will receive email confirmation from the CE department when you have been registered for the course and/or travel.
