If you cancel an internal CE course, the CE Department must receive notification at least 72 hours prior to course date and check in time or you will incur a $25.00 administrative fee. (Eg: If course taking place on a Saturday at 8 am, CE department must receive cancellation by Wednesday morning at 8 am). 

The fee will be charged in all situations but discretion will be used if the reason is due to a real hardship (death in the family, emergencies, etc.). Directors will be notified of any cancellations within the 72 hour time frame so they are aware.

If you owe a cancellation fee, you will not be allowed to register for an internal course or attend an external course until the fee has been paid.

Non-Fairview Employee Cancellation Policy

All cancellations must be submitted in writing via mail, e-mail or fax. For cancellations received 14 days before the seminar date, the seminar fee (if applicable) will be returned, less a $25.00 administrative fee. Within 14 days of the seminar, no refunds will be made, although the seminar fee is transferable to another person for the same seminar date.

Fairview Health Services reserves the right to cancel or reschedule this seminar due to an insufficient number of registrants or other unforeseen circumstances. Under these circumstances, seminar fees will be returned in full to the registrant (if applicable). Please note that Fairview Health Services is not responsible for hotel, airline or other expenses incurred.

M Health Fairview | Rehab Services CEU/Contact Hours and Policy

M HEALTH Fairview is no longer an approved provider through AOTA. For additional information about CE requirements for license,
please see MN Statute 148.6443: https://www.revisor.mn.gov/statutes/cite/148.6443

 

Physical Therapy - courses that have been approved for category I PT credit are not eligible for partial credit. If a PT participant leaves a course early, the CE coordinator must be contacted to remove the Category I approved Language and adjust the contact hours to reflect the time the staff member spent in the course.

Speech Language Pathology -  ASHA approved provider courses will not be granted partially approved credits. If an SLP participant leaves a course early, the CE Coordinator must be contacted to modify the certificate to remove the approved provider status language and adjust the contact hours to reflect the time the staff member spent in the course.

CEU/Contact hours will be issued upon course completion.

Release of Learner Records for Fairview Rehabilitation Continuing Education Department

Continuing Education (CE) records are retained for 7 years. Participants requesting CE records will be asked to provide identifying information to assure the privacy and security of learner’s records. Verification of employee identification number and department information is required prior to release of information. External participant requests are verified by participant’s home mailing address and telephone number.

M Health Fairview  Disclosure Policy

M Health Fairview  requires that all participating speakers, course directors, and planning committee members to disclose to the program audience any financial relationships related to the subject matter of this program. Relationships of a spouse/partner with proprietary entities producing health care goods or services should be disclosed if they are of a nature that may influence the objectivity of the individual in a position to control the content of the continuation education activity. Disclosure information is reviewed in advance in order to manage and resolve any possible conflicts of interest. Specific disclosure information for each speaker, course director, and planning committee member will be shared with the audience prior to the speaker’s presentation.

Copyright Information for Participating Speakers

Speaker(s) warrants that the work that is presented will be his or her own original creation and that it will in no way infringe upon any literary, copyright or proprietary right of any other person or business entity.  Speaker agrees to obtain such permission(s) from the copyright owner(s) should presentation or the material(s) used during or in connection with presentation require permission for use.
